June 2026 Enforcement Action Summary
This report includes formal enforcement actions issued or resolved by the Office of Compliance & Inspection (OC&I) for June 2026. This report is intended to inform the public of certain compliance/enforcement activities performed in the preceding month to protect the public's health, safety, welfare, and the environment.
Formal enforcement actions are taken to achieve compliance with law/regulations; remediate environmental damage; restore natural resources to appropriate conditions; impose penalties that capture the gravity of the situation and any economic benefit gained by the alleged violator; and deter similar actions in the future. Formal enforcement is generally in the form of a Notice of Violation (NOV) that alleges certain facts and violations, contains orders to resolve the alleged violations, contains an assessed penalty with supporting documentation regarding what factors OC&I used to determine the penalty, and, by law, allows a respondent the ability to appeal or contest the NOV to the Rhode Island Department of Environmental Management (RIDEM) Administrative Adjudication Division (AAD). Since most NOVs are contested cases, OC&I does not generally discuss the case with the public while the matter is awaiting hearing or pending negotiated settlement. NOVs are subject to release under the Access to Public Records law in Rhode Island. A copy of an individual NOV may be obtained through the RIDEM's Office of Customer and Technical Assistance. OC&I often resolves formal enforcement actions through negotiated settlement agreements prior to hearing before the AAD.
June 11, 2026 – Hazardous Waste File No. OCI-HW-25-43 re: Northland Environmental, LLC. for property located at 252 & 275 Allens Avenue in Providence, Rhode Island (“Property”). The Property includes a regulated hazardous waste treatment, storage, and transfer facility (“Facility”). Respondent operates the Facility. OC&I alleges that Respondent violated Rhode Island’s Rules and Regulations for Hazardous Waste Management (250-RICR-140-10-1). View the NOV.
June 25, 2026 – Underground Storage Tank File No. OCI-UST-26-16-01263 re: Imad Melhem and GNI West Warwick, Inc. for property located at 929 Providence Street, Assessor’s Plat 39, Lot 213, in West Warwick, Rhode Island (“Property”). The Property includes a petroleum product storage and dispersing system and an automotive service station (“Facility”). Respondent Imad Melhem owns the Property. Respondent GNI West Warwick, Inc. operates/operated the Facility. OC&I alleges that Respondents violated Rhode Island’s Underground Storage Facilities Used for Regulated Substances and Hazardous Materials (250-RICR-140-25-1). View the NOV.
June 29, 2026 – Dam File No. OCI-DAMS-24-25 re: Town of Glocester for the following Dam State I.D. Nos: 022 (Keech Dam), 023 (Smith & Sayles Dam), 029 (Mower Dam), 032 (Snakeskin Dam), 165 (Ponaganset Dam), 354 (Coomer Dam), 401 (Lake Washington Dam), 514 (Lake Aldersgate Dam), 555 (Hawkins Dam), 556 (Clarkville Dam), 587 (Wright T. Farm Dam), 594 (David King Farm Dam), and 727 (Bowdish Lower Dam) (collectively the “Dams”). The Dams are located within the Town of Glocester, Rhode Island. Keech Dam, Ponaganset Dam, Lake Washington Dam, Hawkins Dam, Clarkville Dam and Bowdish Lower Dam are classified by RIDEM as High Hazzard. Smith & Sayles Dam, Mower Dam, Snakeskin Dam, Coomer Dam, Lake Aldersgate Dam, Wright T. Farm Dam, and David King Farm Dam are classified by RIDEM as Significant Hazard. Nos: 127 (Belknap Dam), 168 (Oak Swamp Dam), 169 (Almay Dam), 170 (Simmons Upper Dam), 310 (Pocasset Dam), 313 (Hughesdale Dam), 323 (Caesarville Dam), 346 (Kimball Dam), and 504 (Dexter Dam) (collectively the “Dams”). The Dams are located within the Town of Johnston, Rhode Island. Oak Swamp Dam, Almy Dam, Simmons Upper Dam, and Hughesdale Dam are classified by RIDEM as High Hazard. Belknap Dam, Pocasset Dam, Caesarville Dam, Kimball Dam and Dexter Dam are classified by RIDEM as Significant Hazard. Nos: 043 (Slatersville Upper Dam), 046 (Slatersville Middle Dam). 047 (Slatersville Lower Dam), 048 (Forestdale Dam), 068 (Woonsocket #3 Dam), and 070 (Woonsocket #1 Dam) (collectively the “Dams”). The Dams are located within the Town of North Smithfield, Rhode Island. Slatersville Upper Dam, Slatersville Middle Dam, Forestdale Dam, Woonsocket #3 Dam, and Woonsocket #1 Dam are classified by RIDEM as High Hazard. Slatersville Lower Dam is classified by RIDEM as Significant Hazard. OC&I alleges that Respondent violated Rhode Island’s Inspection of Dams and Reservoirs Act (R.I. Gen. Laws § 46-19-9). June 11, 2026 – Underground Storage Tank File No. OCI-UST-26-7-04759 re: 2340 West Main Realty LLC and Patriot West LLC for property located at 2340 West Main Road, Assessor’s Plat 56, Lot 6 in Portsmouth, Rhode Island (“Property”). The Property includes a convenience store and a petroleum product storage and dispensing system (“Facility”). Respondent 2340 West Main Realty LLC owns the Property. Respondent Patriot West LLC operates the Facility. On May 11, 2026, OC&I issued a NOV to Respondents alleging that Respondents violated Rhode Island’s Underground Storage Facilities Used for Regulated Substances and Hazardous Materials (250-RICR-140-25-1). View the NOV. Respondents did not file an appeal of the NOV with AAD. Respondents complied with the Order section of the NOV and paid a reduced penalty of $1,500 to fully resolve the NOV.

The Property includes a petroleum product storage and dispensing system (“Facility”). Respondent Countryside LLC owns the Property. Respondent Village Gas 4 Inc. operates the Facility. On April 2, 2026, OC&I issued a NOV to Respondents alleging that Respondents violated Rhode Island's Rules and Regulations for Underground Storage Facilities Used for Regulated Substances and Hazardous Materials (250-RICR-140-25-1). View the NOV. Respondents did not file an appeal of the NOV with AAD. Respondents complied with the Order section of the NOV and paid a reduced penalty of $1,500 to fully resolve the NOV.
June 17, 2026 – Freshwater Wetlands File No. OCI-FW-24-163 re: The Vincent R and Colleen M. Graziano Revocable Trust for property located approximately 100 feet south of Henry Brown Road; approximately 2,300 feet east of the intersection of Henry Brown Road and Orion View Drive: utility Pole Nos 36 and 37, Assessor’s Plat 52, Lot 2-2, with an address of 280 Henry Brown Road, in West Greenwich, Rhode Island (“Property). Respondent owns the Property. On November 13, 2025, OC&I issued a NOV to Respondent alleging that Respondent violated Rhode Island’s Freshwater Wetlands Act, and the Regulations Governing the Administration and Enforcement of the Freshwater Wetlands Act (250-RICR-150-15-3). View the NOV. Respondent filed an appeal of the NOV with AAD. Prior to an administrative hearing on the NOV, OC&I and Respondent executed a Consent Agreement to resolve the NOV. View the Agreement.
June 25, 2026 – Air Pollution File No. OCI-AIR-26-12 re: Herrick & White Ltd. for a facility located at 3 Flat Street in Cumberland, Rhode Island (“Property”). Respondent operates a custom woodworking facility on the Property (“Facility”). On March 30, 2026, OC&I issued a NOV to Respondent alleging that Respondent violated Rhode Island’s Air Pollution Control Regulations, Part 9 titled Air Pollution Control Permits (250-RICR-120-05-9) and Part 29 titled Air Pollution Control Regulations No. 29 – Operating Permits (250-RICR-120-05-29). View the NOV. Respondent filed an appeal of the NOV with AAD. Respondents complied with the Order section of the NOV and paid a reduced penalty of $17,500 to fully resolve the NOV.
July 21, 2025 – Air Pollution File No. OCI-AIR-25-11 re Technic, Inc. for properties located at 41 Century Drive and 601 Park East Drive, Woonsocket, Rhode Island (“Facilities”). Respondent operates both Facilities. On July 21, 2025, OC&I issued a NOV to Respondent alleging that Respondent violated Rhode Island’s Air Pollution Control Regulations titled Air Pollution Control Permits (250-RICR-120-05-9) and Emission of Air Contaminants Detrimental to Person or Property (250-RICR-120-05-7.6). View the NOV. Respondent filed an appeal of the NOV with AAD. Prior to an administrative hearing on the NOV, OC&I and Respondent executed a Consent Agreement to resolve the NOV. View the Agreement.
June 29, 2026 – Underground Storage Tank File No. OCI-UST-23-6-00431, and Superior Court Case File No. PC-2026-03483 re: Plaintiff Terrance Gray, Director, RIDEM, v. Jeha Corp, JOHN JEHA, INC., and John Jeha for a property located at 76 Newport Avenue, Assessor’s Plat 40, Lot 1008, in Pawtucket, Rhode Island. On September 20, 2023, OC&I issued a NOV to Respondents alleging that Respondents violated Rhode Island's Rules and Regulations for Underground Storage Facilities Used for Regulated Substances and Hazardous Materials (250-RICR-140-25-1). View the NOV. Respondents filed an appeal of the NOV with AAD. On June 3, 2025, AAD issued a Decision and Order upholding the NOV in its entirety. View the Decision. On June 29, 2026, RIDEM filed a complaint in Superior Court against the Defendants asking the Court to order the Defendants to comply with the NOV. View the Complaint.
